Why Doesn't Water Help Spicy Food? The Fat and Sugar Behind the Fix

One bite too many and your mouth is on fire, and the first thing your hand finds is the water glass. You drink, it is briefly cold, and the burn is still exactly where it was. Why doesn't water help spicy food? The reason is chemistry, not willpower, and what actually helps is something fatty, a sip of milk or a spoon of yogurt, with a little sweetness alongside it.

The burn is a pain signal, not a taste

Chili heat does not appear anywhere on the list of things the tongue is built to read, which leaves an obvious question about what you are actually feeling when a pepper goes off in your mouth.

Chili heat is not a taste but a pain signal: capsaicin binds and opens the TRPV1 ion channel on pain-sensing neurons, letting sodium and calcium flow in to depolarize the neuron and fire the burning sensation, the same channel that noxious heat activates.1

Your body is reporting an injury that never happened. That also explains why the usual reflex misses the target: putting out this fire is not a matter of pouring water on it, but of getting a molecule off the nerve endings it has latched onto.

Capsaicin is strongly lipophilic and almost insoluble in water, which is why rinsing with water fails to wash it off the TRPV1 receptors while fatty foods dissolve and carry it away.2

This is why a sip of milk does what a whole glass of water cannot, and why the water goes past the burn without taking any of it along. The property is not unique to chili heat, either.

Why a little sweetness helps too

If fat is the move that carries the molecule out, what is it that sweetness does?

Adding sweetness measurably lowers perceived chili burn, an effect attributed to sweet-taste input dampening pain transmission rather than to sugar chemically neutralizing capsaicin.4

Notice the wording: the sugar is not removing the capsaicin, it is quieting how strongly the burn registers, which is a different target entirely. Fat carries the molecule out, and sweetness turns down the volume.
